Not saying I can make them a better golfer, but I can help relieve some of the pain they are experiencing. Every day, I meet patients who express, ‘Here’s my problem, can you help me? Then together, with an agreed upon plan, we work towards a treatment plan that suits them. It’s incredibly rewarding to help someone regain their quality of life."

Joint Replacement vs. Joint Reconstruction

A common point of confusion for patients is the difference between joint replacement and joint reconstruction. Dr. Assini clarifies, "In many cases, these terms are used interchangeably, and you can also throw in joint arthroplasty. The goal of a joint replacement procedure is to relieve pain and restore function." Joint replacement typically involves replacing the damaged joint with an artificial one. These implants are typically made of metals and plastics and resurface the damaged cartilage. “Before offering a joint replacement I try to understand the specific needs, activities and expectations of my patients. This can affect the type of implant we use, or what technology we use to implant it.

"I always emphasize the importance of clear communication. It’s vital that patients understand what to expect from surgery and how it can impact their quality of life." He says that his most successful patients follow their post-surgery recommendations, have their home ready, and have a support system in place to recover. He mentions the importance of mentally preparing for the possibility of having to adjust activity levels post-surgery to accommodate the new joint and allow it time to heal. Rapid recovery total joint replacement is different for everyone, and an individual approach is always best.

For those contemplating orthopedic surgery, Dr. Assini offers practical advice. "It’s essential to consider both the short-term and long-term perspectives of surgery." He encourages potential patients to optimize their health before the procedure—focusing on weight management, exercise, nutrition, and hydration can make a substantial difference. "A good setup at home, coupled with proper nutrition and hydration, can significantly enhance the recovery experience."

A Complex Case: A Journey to Recovery

When asked about his most complex case, he quickly shared that he is most proud of the 99% of the people he helps to enjoy a better life through the new ability to move without pain. Dr. Assini reflects on one of his most challenging cases involving a patient who suffered a severe femur fracture after a collision and came to him after 6 previous surgeries on their leg. This patient had already been on a long road to recovery, spending months in a wheelchair and undergoing extensive rehabilitation. "The benefits of surgery for my patient were life changing. We were able to reconstruct the bottom of their femur and knee joint. The patient was able to walk within a few weeks and now after a year they are completely mobile and have their independence back. There is a significant amount of trust patients put with their surgeon, and we need to make sure we recognize that fact and do all we can to improve quality of life."

The Role of Rapid Recovery Total Joint Protocol and AnteriorPath

One of the most exciting advancements in orthopedic care is the Rapid Recovery Total Joint Protocol. Dr. Assini explains how this approach revolutionizes recovery for patients. "The goal is to get patients up and moving as soon as possible post-surgery. By utilizing targeted anesthesia, getting the muscles around the joint moving, and providing a structured rehabilitation plan; we can help patients regain function quickly. This gets my patients back to enjoying their life with less pain, sooner."

Dr. Assini and a team of five surgeons developed an evolutionary procedure called AnteriorPATH, which is a new way to do anterior total hip replacement. AnteriorPATH uses a newly designed instrument to place a cannula (think of a BIC pen) through a small 1 cm incision on the patient’s thigh. The cannula is then used as a portal to the hip joint. This in turn allows Dr. Assini to move the surgical incision into a more favorable location in the groin. The ‘bikini’ incision is located in the flexion crease of the groin and is known to heal more readily, reduce nerve injury, and allows better visualization for component placement. A faster healing incision reduces infection risk and better component placement leads to improved pain relief and joint function.

AnteriorPATH was introduced in 2019 and is an important step forward in total hip evolution. Dr. Assini and the other surgeon designers are completing a Multicenter Trial to quantify and publish the benefits for patients.
