One of our priorities in 2018 is transitional care for medically complex patients who are aging out of pediatrics into adult services. With a skyrocketing population of pediatric patients with complex medical conditions, this is a critical next step. Adler Family Coordinated Care for Kids (AFCCK) staffs a physician, three nurse care coordinators, two social work care coordinators, a program assistant, a medical assistant and a program manager. The program’s Complex Care Clinic provides tertiary level care to its pediatric patients in an outpatient setting with the goal of keeping them out of the hospital while receiving appropriate medical services and support.
AFCCK was a recipient of the 2016 Adventist Health System Medicine Innovation Award, given to a hospital medicine group that has conceived and implemented an innovation in care that brought significant value to patients in the community. Florida Hospital for Children has been a leader in bringing new programs to children throughout the state. The AFCCK program is another unique and innovative approach to a very complex group of pediatric patients who need special care.
