The PGY1 Pharmacy Residency Program at AdventHealth Shawnee Mission is designed to develop proficiency in a variety of clinically oriented patient care settings. Our mission is to integrate the pharmacy practice resident with a variety of disciplines within the patient care team.

Anticoagulation Management Service
Staffed by clinical pharmacists specializing in anticoagulation management.
What to expect:
Patients are scheduled for 30-minute office visits during which an instant finger-stick INR is obtained as well as an assessment of therapy. Our clinical pharmacists provide the patient with dosing instructions and may renew prescriptions for warfarin and low-molecular-weight heparins on behalf of the patient's enrolling physician as part of hospital policy. Patients leave each visit with instant INR results, a dosing regimen, and a scheduled follow-up appointment. An official report with patient's INR result, assessment and dosing instructions are faxed or available in EPIC within 48hrs.
How to enroll:
Patients may enroll by having their physician complete and return this enrollment form. Once complete, fax the form to Call913-218-1119. Please be advised that only KS-licensed physicians may refer patients.
